Included Plans
PPO and POS plans with the Claritev logo are usually accepted.
Excluded Plans
Medicare/Medicaid, and non-PPO are typically excluded.
Confirmation Required
Coverage is confirmed only after you complete the coverage check.
Visit Allowance
Visit numbers vary by plan. Coverage is confirmed during eligibility verification.
Provider Type
